AI symptom checkers vs. physician diagnosis: how big is the accuracy gap, really?
Large-scale comparison studies have quantified how often AI triage tools get the right diagnosis on the list — and how often they don't.
Independent studies testing AI-based symptom checkers against physician diagnosis for the same case vignettes have found wide variance between individual tools — some place the correct diagnosis within their top-three suggestions a majority of the time, others considerably less often, and accuracy tends to drop further for less common conditions and for cases with atypical symptom presentation.
The more consistent finding across these comparisons is that AI tools tend to be appropriately cautious on urgency — correctly flagging when a symptom combination warrants urgent care — more reliably than they nail the specific diagnosis. That's arguably the more important function for a triage tool: 'this needs evaluation soon' matters more than a precise label the tool can't fully verify anyway.
The responsible use case that's emerged from this research is pre-visit triage and information-gathering, not diagnosis replacement — using the tool to organize symptoms and decide urgency before seeing a clinician, rather than treating its output as a diagnosis to act on independently.
