Altitude training masks: what they simulate, and what they don't
A resistance mask that makes breathing harder is often marketed as replicating high-altitude training. The physiology behind that claim doesn't hold up.
Altitude training masks restrict airflow, making each breath require more effort — that's genuinely what they do. What they don't do, despite common marketing framing, is replicate the actual physiological stress of training at altitude, which comes specifically from reduced oxygen partial pressure in the air, not from mechanical breathing resistance. A mask makes breathing muscles work harder against resistance; it doesn't reduce the amount of oxygen available in each breath the way thinner air at elevation does.
This distinction matters because altitude's real training benefits — increased red blood cell production, improved oxygen-carrying capacity over weeks of exposure — are driven by the hypoxic (low-oxygen) stimulus specifically, and a resistance mask doesn't create meaningful hypoxia; blood oxygen saturation in mask users during exercise generally stays close to normal, unlike genuine altitude exposure where it measurably drops. Studies directly comparing mask training to real or simulated altitude training consistently find the mask doesn't replicate altitude's key adaptations.
What a resistance mask can plausibly do is train the respiratory muscles themselves — the diaphragm and accessory breathing muscles — similar in concept to dedicated inspiratory muscle trainers, which do have some trial support for improving breathing endurance in specific populations, including some benefit shown for athletes and for certain respiratory conditions. That's a real but much narrower benefit than 'altitude simulation.'
For someone specifically trying to replicate altitude's cardiovascular adaptations, an altitude mask isn't the tool — genuine altitude training, altitude tents, or hypoxic training chambers are the interventions with the physiological mechanism to back the claim. As a respiratory muscle training tool with a much more modest, specific benefit, a resistance mask has some legitimate use, but it's worth choosing it for the right reason rather than the altitude-simulation marketing.
