DHEA supplements: what the hormone actually does, and where the evidence runs out
DHEA production declines steadily with age, which has fueled decades of supplementation interest. The actual trial evidence for restoring it is considerably more limited than the supplement's popularity suggests.
DHEA (dehydroepiandrosterone) is a hormone produced primarily by the adrenal glands that serves as a precursor the body converts into testosterone and estrogen; its production peaks in early adulthood and then declines steadily and substantially with age, a decline steep and consistent enough that it's been proposed as a potential biomarker of aging itself, and one that's fueled decades of interest in whether restoring youthful DHEA levels through supplementation could meaningfully counter aspects of age-related decline.
Trial results on DHEA supplementation in older adults have been genuinely mixed rather than consistently supportive, despite the compelling age-decline pattern that motivated the research interest — some trials have shown modest improvements in bone density, skin thickness, or sexual function in specific populations, particularly older adults with clearly documented low DHEA levels, while other well-designed trials looking at broader outcomes like muscle strength, cognitive function, or overall quality of life in general older adult populations have found no significant benefit over placebo.
That inconsistency across trials likely reflects real biological complexity rather than simply poor study design — DHEA's downstream effects depend heavily on how it's actually converted to testosterone or estrogen in a given individual's tissues, which varies by sex, age, and individual metabolic factors in ways that aren't fully predictable, meaning a supplementation approach that works well for one person's hormone conversion pattern may do little for another's.
Given the mixed trial evidence, DHEA supplementation is a reasonable thing to discuss with a physician specifically for documented low levels with corresponding symptoms, ideally with baseline and follow-up hormone testing to confirm both the initial deficiency and the supplement's actual effect — but it doesn't currently have strong enough evidence to recommend as a general anti-aging intervention for older adults without a documented deficiency, despite how intuitively appealing the age-decline pattern makes it seem.
