Exercise-induced bronchospasm: why some lungs react to exertion, and how it's managed

Exercise-induced bronchospasm — airway narrowing triggered specifically by physical exertion, producing coughing, wheezing, chest tightness, or breathlessness during or shortly after exercise — is a distinct, well-characterized condition that can occur in people with diagnosed asthma and, less commonly, in people with no other asthma symptoms at all, triggered specifically by the combination of rapid breathing and, in many cases, cooler or drier air moving quickly through the airways during sustained exertion.
The proposed mechanism centers on airway drying and cooling: normal breathing warms and humidifies air before it reaches the lower airways, but the rapid breathing rate during vigorous exercise can outpace this conditioning process, and the resulting airway drying triggers the same inflammatory bronchoconstriction response seen in other asthma triggers — which is why exercise-induced bronchospasm tends to be worse in cold, dry conditions and comparatively milder in warm, humid environments, including pools, for the same humidity-related reason relevant to COPD exercise.
Diagnosis typically relies on a pattern history — symptoms reliably appearing during or just after exertion and resolving with rest — sometimes confirmed with specific exercise or bronchial challenge testing when the diagnosis is unclear from history alone, since a range of other conditions (including vocal cord dysfunction, which can look similar) can mimic the same exertional symptom pattern and benefit from being ruled out with proper testing.
Management is generally straightforward and effective once diagnosed: a short-acting bronchodilator used specifically before exercise, as a preventive measure rather than only as a rescue treatment after symptoms start, prevents the bronchospasm in most patients, and a proper warm-up period before higher-intensity exercise has independent supporting evidence for reducing symptom severity. For most diagnosed patients, this condition doesn't mean giving up vigorous exercise — it means a specific, manageable pre-exercise routine.
