Choosing a home blood pressure monitor: the accuracy differences that actually matter
Not all consumer blood pressure monitors are equally accurate, and the differences that matter most aren't the ones most shoppers check first.
Home blood pressure monitoring has real clinical value — it captures readings in a familiar environment without the 'white coat effect' that inflates readings in a clinical setting, and it provides the kind of longitudinal data a single office visit reading can't — but the accuracy of the specific device matters more than most shoppers account for when comparing options primarily on price or brand recognition alone.
Cuff size is the single most common accuracy problem in home monitoring, and it's almost entirely avoidable: a cuff too small for the user's arm circumference produces artificially elevated readings, sometimes significantly so, while a cuff too large can produce artificially low readings — most monitors ship with a standard cuff sized for an average adult arm, and anyone with an arm circumference meaningfully larger or smaller than that average needs a correctly sized cuff, not the one that happened to come in the box, for genuinely accurate readings.
Validation status is the accuracy signal worth checking before purchase and the one most easily overlooked: independent validation programs (including those recognized by major hypertension and cardiology organizations) test specific monitor models against a clinical reference standard and publish which models pass — checking a specific model against these validation lists is a more reliable accuracy signal than price point, brand reputation, or marketing claims about 'clinical accuracy' printed on the box itself.
Upper-arm monitors are generally considered more consistently accurate than wrist monitors across validation studies, primarily because wrist position relative to heart level is more variable and more error-prone for an untrained user to get consistently right compared to an upper-arm cuff, which is more forgiving of minor positioning variation — for anyone prioritizing accuracy over the convenience wrist monitors offer, this is a meaningful practical consideration.
