Home sleep study vs. in-lab polysomnography: which one your symptoms actually call for
A home test is cheaper, faster, and more convenient — but it isn't the right tool for every sleep complaint, and knowing the difference saves a repeat test later.
A home sleep apnea test uses a simplified sensor set — typically airflow, oxygen saturation, and breathing effort — worn overnight in the patient's own bed, and is validated specifically for diagnosing moderate-to-severe obstructive sleep apnea in patients with a straightforward clinical presentation and no significant competing conditions. In-lab polysomnography adds brain-wave monitoring, muscle activity, eye movement, and continuous technician oversight, producing a far more complete picture of what's actually happening across sleep stages.
That completeness matters for a specific set of situations home testing isn't built for: suspected central sleep apnea rather than obstructive, significant heart failure or chronic lung disease that complicates the breathing pattern, suspected REM sleep behavior disorder or other parasomnias that require watching for abnormal movement during specific sleep stages, or a home test that came back negative despite strong clinical suspicion of sleep apnea — a false-negative home test is common enough that a negative result in a high-suspicion patient usually warrants the in-lab follow-up rather than ruling out apnea entirely.
Cost and access differences are real and part of why home testing has become the default first step for straightforward cases: it's substantially cheaper, doesn't require a sleep lab appointment that can be weeks out, and lets patients sleep in their own environment, which for some people produces a more representative night than an unfamiliar lab setting would.
The practical rule of thumb is that home testing is a reasonable first step for typical snoring-plus-daytime-fatigue presentations without complicating conditions, while anything with a neurological, cardiac, or pulmonary complication — or a home test result that doesn't match the clinical picture — is worth escalating to in-lab testing rather than treating the home result as final.
