Hormone replacement therapy and longevity: where the evidence actually stands
HRT's reputation has swung sharply in both directions over the past two decades. A more careful look at what current, better-designed research actually shows about risk and timing.
Hormone replacement therapy's reputation has swung considerably over the past two decades: widely prescribed in the 1990s on the assumption of broad cardiovascular and longevity benefit, then sharply curtailed after the Women's Health Initiative study raised significant safety concerns in the early 2000s, and more recently subject to renewed, more nuanced reassessment as researchers have re-examined that original study's design and applicability more critically.
The re-examination centers substantially on a factor that got insufficient attention initially: the 'timing hypothesis,' which holds that HRT's risk-benefit profile differs meaningfully depending on how soon after menopause it's started, with more recent analysis suggesting that starting HRT closer to menopause onset carries a different, generally more favorable risk profile than starting it many years later — a distinction the original Women's Health Initiative study's participant population, which included many women starting HRT well after menopause, wasn't well-positioned to fully separate out.
Current guidance from major medical societies has moved toward a more individualized framework rather than either the earlier broad enthusiasm or the subsequent broad caution: for appropriately selected women, generally closer to menopause onset and without specific contraindicating risk factors, HRT's benefits for menopausal symptom management and, for some, bone density preservation are considered to reasonably outweigh risks for many patients, while the calculation looks different for women starting later or carrying specific cardiovascular or cancer risk factors.
The honest state of the longevity-specific question — whether HRT meaningfully extends lifespan or healthspan beyond its more established symptom and bone-density benefits — remains less settled than either the earlier enthusiasm or the subsequent alarm suggested, and this is a decision genuinely worth working through individually with a physician familiar with a patient's specific risk profile and the current, more nuanced evidence, rather than applying a blanket rule based on either the 1990s or early-2000s framing of the question.
