Lipoprotein(a): the cardiovascular risk factor most people have never been tested for
Lp(a) is largely genetically determined, isn't affected by diet or exercise, and isn't part of a standard lipid panel — here's why it matters anyway.
Lipoprotein(a), or Lp(a), is a genetically determined particle whose blood levels are set almost entirely by inheritance and remain largely stable throughout adult life, unlike LDL cholesterol which responds substantially to diet, exercise, and statins. Elevated Lp(a) is an independent risk factor for cardiovascular disease and aortic valve stenosis, meaning it raises risk even when LDL and other standard markers look normal.
It's not included in a routine lipid panel by default, which means a substantial number of people at elevated cardiovascular risk from this specific factor are never identified — cardiology guidelines increasingly recommend at least one lifetime Lp(a) test for most adults, since a single measurement is generally sufficient given how stable the levels are over time.
Until recently there was little to do about an elevated result beyond more aggressive management of the risk factors that are modifiable (LDL, blood pressure, smoking). That's changing: several Lp(a)-lowering drugs are in late-stage clinical trials, which is part of why testing now, even without a targeted treatment yet available, is increasingly recommended — knowing your number positions you to act as soon as a targeted therapy is approved.
