Fish oil supplements and heart health: what the largest cardiovascular trials actually found
Omega-3 supplementation's cardiovascular benefit is more contested in large trial data than the supplement aisle suggests.
Large randomized cardiovascular outcome trials testing standard fish oil supplements against placebo have produced notably mixed results — several major trials found no significant reduction in cardiovascular events from standard-dose omega-3 supplementation in general or moderate-risk populations, which complicated the earlier, more optimistic picture built largely from observational studies of fish consumption.
One prominent trial using a high-dose, purified EPA formulation (rather than a standard mixed EPA/DHA supplement) did find a significant cardiovascular event reduction in a high-triglyceride, elevated-risk population — a result that's been influential but is specific to that formulation, dose, and population, and hasn't been shown to generalize cleanly to standard over-the-counter fish oil supplements taken by lower-risk individuals.
The distinction the current evidence supports is between eating fatty fish as a whole food (which retains observational support for cardiovascular benefit) and taking a generic fish oil capsule expecting the same outcome, which large trial data doesn't strongly support for general primary prevention — a nuance that's often lost in supplement marketing built on the earlier observational research.
