Retinoids — vitamin A derivatives including prescription tretinoin and over-the-counter retinol — have among the most extensive clinical trial evidence of any topical anti-aging ingredient, with decades of dermatology research demonstrating measurable improvements in fine lines, skin texture, and pigmentation through mechanisms including increased collagen production, accelerated cell turnover, and improved dermal structure visible on histological analysis, not just self-reported appearance.
Prescription-strength tretinoin has the strongest evidence base specifically, since it's been the subject of the most rigorous, longest-running dermatology trials — over-the-counter retinol requires conversion to retinoic acid within the skin to become active, making it meaningfully less potent than prescription tretinoin at an equivalent labeled concentration, which matters for setting realistic expectations from an OTC product.
