Why cancer screening isn't done every year: the logic behind the intervals

Cancer screening intervals longer than a year — three years for cervical cancer screening in many current guidelines, ten years for colonoscopy in average-risk patients, for instance — feel counterintuitive to a lot of patients who reasonably assume more frequent testing is straightforwardly safer, but the actual interval recommendations are grounded in specific reasoning about cancer growth rates and the real costs of over-frequent testing that's worth understanding rather than assuming longer intervals reflect reduced caution.
The core logic starts with how quickly a given cancer type typically progresses from an undetectable or pre-cancerous state to a clinically significant, symptomatic stage: this progression timeline varies considerably by cancer type, and screening intervals are calibrated specifically to that biology — a slower-progressing cancer type reasonably supports a longer screening interval than a faster-progressing one, since the screening test has a genuinely wider effective window in which to catch it before it advances significantly.
More frequent screening than the biological progression timeline actually requires doesn't meaningfully improve detection of the cancer type being screened for, but it does meaningfully increase the cumulative burden of false positives, unnecessary follow-up procedures, and the overdiagnosis risk discussed elsewhere across a patient's lifetime — this is the less intuitive but genuinely important half of the interval calculation: unnecessarily frequent testing carries real costs and risks of its own, not simply added reassurance at no downside.
Screening intervals are also periodically revised as new evidence emerges about disease progression timelines and screening test performance, which is why guideline intervals do sometimes change over time — a change to a longer interval, as happened with cervical cancer screening following the adoption of HPV testing discussed elsewhere, generally reflects updated evidence about test accuracy and disease biology, not a reduced institutional commitment to cancer prevention.
