Salt sensitivity isn't universal — the individual variation research behind sodium guidelines
Blood pressure response to sodium intake varies significantly between individuals, and genetics and other factors explain a meaningful part of why.
Population-level sodium reduction guidelines are built on average effects across large groups, but individual blood pressure response to dietary sodium varies considerably — research on 'salt sensitivity' has found that a meaningful subset of the population shows a pronounced blood pressure response to sodium intake changes, while others show comparatively little change even with substantial intake variation.
Factors associated with higher salt sensitivity in the research include age (sensitivity tends to increase with age), African ancestry (population studies have found higher average salt sensitivity), obesity, chronic kidney disease, and diabetes — meaning the guideline that applies most strongly to a given individual depends partly on which of these categories they fall into, rather than a single universal sodium target being equally important for everyone.
This doesn't undermine general sodium reduction guidance for the population overall — the average effect across large groups remains real and clinically meaningful for cardiovascular risk. It does mean that for people who don't fall into higher-sensitivity categories and who don't have hypertension, the individual stakes of moderate sodium variation are somewhat lower than population-wide messaging sometimes implies, which is a distinction worth a direct conversation with a physician rather than assuming a single number applies identically to everyone.
