Vagus nerve stimulation: separating the wellness claims from the clinical evidence
Vagus nerve stimulation has genuine, FDA-approved medical uses. The wellness-market claims attached to consumer devices and 'vagal toning' exercises go considerably further than that evidence supports.
The vagus nerve is a major nerve running from the brainstem through the neck and chest into the abdomen, playing a central role in the parasympathetic ('rest and digest') nervous system, regulating heart rate, digestion, and inflammatory response among other functions. Vagus nerve stimulation as an actual medical treatment — using an implanted device to deliver electrical pulses to the nerve — has genuine, FDA-approved clinical applications for treatment-resistant epilepsy and treatment-resistant depression, with real trial evidence supporting effectiveness for these specific, difficult-to-treat conditions.
That established clinical use is meaningfully different from the much broader wellness-market framing around 'vagal toning' — breathing exercises, cold exposure, humming, and consumer transcutaneous stimulation devices marketed as ways to improve heart rate variability, reduce stress, and support broad health and longevity through vagus nerve activation. Some of these practices do have real, measurable short-term effects on markers linked to parasympathetic activity, like heart rate variability, but the leap from 'measurably shifts a physiological marker in the moment' to the sweeping longevity and chronic disease claims often attached to these practices runs well ahead of the actual evidence.
Consumer transcutaneous vagus nerve stimulation devices — worn on the ear or neck, distinct from the implanted medical devices used for epilepsy and depression — have a much smaller and more preliminary evidence base, with some small studies suggesting modest effects on stress and inflammatory markers, but nothing approaching the scale of evidence behind the implanted medical devices' approved clinical indications.
For general stress management, practices associated with vagal activation — slow diaphragmatic breathing, cold exposure, and others — are reasonable, low-risk things to incorporate given their measurable short-term physiological effects, without needing to accept the more expansive longevity and disease-prevention claims sometimes attached to them. For actual epilepsy or treatment-resistant depression, vagus nerve stimulation as an FDA-approved medical device treatment is a genuinely different and much better-evidenced intervention than any consumer wellness device or practice.
