VO2 max as a longevity predictor: what the largest cohort studies actually found
Cardiorespiratory fitness has emerged as one of the strongest independent predictors of mortality risk in large population studies — stronger than several traditional risk factors.
A large cohort study tracking cardiorespiratory fitness (measured via exercise testing, correlating to VO2 max) against all-cause mortality found that fitness level was one of the strongest predictors in the dataset — stronger than smoking status, diabetes, or hypertension as individual risk factors, with the highest-fitness group showing substantially lower mortality than the lowest-fitness group even after adjusting for other risk factors.
What made the finding notable beyond the magnitude was the dose-response pattern at the high end: unlike some risk factors where benefit plateaus, mortality risk kept declining at the very highest fitness levels (elite performance category) rather than flattening out at 'above average,' suggesting there isn't an obvious ceiling where more fitness stops mattering.
VO2 max itself requires a graded exercise test to measure precisely, but it correlates reasonably well with more accessible proxies — performance on a timed run, or increasingly, wearable-estimated VO2 max, though the wearable estimates carry meaningfully wider error margins than a lab test. For most people, the actionable version of this research isn't obsessing over the exact number but engaging in the kind of sustained, progressively challenging cardio training that reliably improves it.
